Grade reports (duplicates or access to), entrance/placement test results, official communications relating to students, probation and dean’s lists, registration schedules, and degree audits.
Admission Office Admission applications, high school and college transcripts, letters of recommendation, and college entrance test scores. Application, transcripts are forwarded to the Office of the Registrar upon initial registration. Test scores are forwarded to the advising coordinator. Grade point average, test scores, class rank summary, and recommendations are used for Admission Committee meetings.
Business Office Name, address, and student identification numbers, student billing information, major.
Department chairs, graduate program coordinators Grade reports (copies or access to), high school record, ACT and SAT profiles, recommendations, transfer evaluations, and degree audit. Academic records are accessible to deans.
Director, Graduate Studies in Education Admission applications, college transcripts, letters of recommendation, and entrance test scores. Application, transcripts are forwarded to the Office of the Registrar upon initial registration (semester based programs) or acceptance (non-semester based degree and licensure programs). Copies may be retained by office. Grade reports (copies or access to), registration schedules, and degree audit. Academic records are accessible to director and designees.
Director, Graduate Studies in Nursing Admission applications, college transcripts, letters of recommendation, and entrance test scores. Application, transcripts are forwarded to the Office of the Registrar upon initial registration. Copies may be retained by office. Grade reports (copies or access to), registration schedules, and degree audit. Academic records are accessible to director and designees.
Director, Graduate Studies in Servant Ldshp. Admission applications, college transcripts, letters of recommendation, and entrance test scores. Application, transcripts are forwarded to the Office of the Registrar upon initial registration. Copies may be retained by office. Financial Aid Office Name, address, and student identification numbers, student financial aid information, academic record information in determining satisfactory progress, major.
Major advisers Grade reports (copies or access to), high school record, ACT and SAT profiles, recommendations and transfer evaluation. Academic records are accessible to advisers.
                     
Office of Career Planning and Placement Personal data sheet, summary of courses completed, letters of recommendation, and any other items that students wish made part of the placement file.
Office of Global Education Admission applications, college transcripts, letters of recommendation, and entrance test scores. Application and transcripts are forwarded to the Office of the Registrar upon initial registration. Test scores are forwarded to the advising coordinator. Copies may be retained by office. INS forms.
Office of the Registrar Permanent academic records, admission application, high school and college transcripts, registration schedules and transaction documents, official correspondence, change/declaration of major, minor, emphasis, degree requirement substitutions and waivers, continuing education records, independent study contracts, credit by examination results, withdrawal from college records, and INS forms.
School Deans Grade reports (copies or access to), high school record, ACT and SAT profiles, recommendations, transfer evaluation, and degree audit. Academic records are accessible to deans.
School of Graduate Studies Dean
Admission applications, high school and college transcripts, letters of recommendation, and college entrance test scores. Application and transcripts are forwarded to the Office of the Registrar upon initial registration.
Vice President for Student Development Personal interest sheet, communications from faculty and administration pertinent to student affairs.
