2.Request official copies of transcripts from all colleges and universities attended (transcripts must be mailed directly from each school.) Transcripts marked "issued to student" will not be accepted. This Transcript Request Form may be used.  
3.Copy of teaching license
4.Completed Written Narrative Form
5.Completed Promissory Note 
6.Completed Check Sheet

Degree Candidacy:Admission to the master’s degree program does not assure acceptance to candidacy for the degree. To be eligible for degree candidacy the applicant must have fulfilled or will fulfill the following requirements:

Fees and Tuition:Fees and tuition may change annually. The fees and tuition listed below are in effect from September 1, 2012 through August 31, 2013.

Application Fee: A non-refundable fee of $50 is required to process the application and must accompany the application for admission to the graduate program.  Students submitting an application for both a licensure program and the Master's degree, at the same time, submit only one $50 application fee.

Comprehensive Exam Fee: A fee of $50 is required with the registration for the Comprehensive Exam.

Graduation Fee: A graduation fee of $65 is payable at the time students register for EDUC 604 Pro-seminar, which is taken immediately prior to graduation.

Tuition for Graduate Courses: Tuition is $310 per graduate credit in Wisconsin and Iowa. Payment is required in full prior to or at the beginning of the course. 

Technology Fee: A fee is added to the Educ 600 course and to the Educ 601 course when taken in La Crosse.

Cancellation of Registration for Elective Courses: Students who cancel registration in writing or by telephone within three days prior to the beginning of classes will be given a full refund of tuition or deposit.

Grade Reports and Transcripts: Grades are processed and grade reports are issued within two weeks after they have been received from the instructor. If a transcript is needed by a specific date, students must plan on a period of at least six weeks and select a course that meets within a timeframe that will make it possible for the transcript to become available. Transcript requests from Viterbo University must be submitted in writing and should be mailed with $5.00 directly to: Office of the Registrar, Viterbo University, 900 Viterbo Drive, La Crosse, WI 54601. Telephone, fax, or email requests will not be accepted per federal regulations. If a student is applying for an add-on license at the same time they are renewing an original license it is recommended that they send both the new and renewal applications in at the same time and MUST included two application fees.
